Yes, that's right -- 'Would you mind giving ...' is the correct form here. The verb 'mind' is followed by a noun. The noun form of a verb is the -ing form, a.k.a. a gerund.

What is the difference between video and movie?
I mostly use video for a short movie, but people in US use movie even for a short video.
Which is correct?

A good place to start for questions like this is the dictionary. There's quite a bit of variation in how people use these words, and they have different meanings in different contexts, but I'd say that 'video' isn't usually used for short films (or 'movies', which means the same thing as 'films' most of the time). I, at least, would call what's on this page a 'video'. The same is true for what YouTube is full of, what you can watch at TED.com, etc.

I don't think there is a clear difference here. The second form is perhaps more formal. However, in terms of politeness the tone of voice, facial expression and so on are more important.
