Great -- thanks for writing such a complete answer! And yes, D is the best answer -- as you say, none of the other three are appropriate, as they answer more a question like 'What are you doing?'. 'I'm doing well' is informal or neutral, so it is appropriate in this context.

I'm not sure if this will interest you or not, but if you want to learn more of this kind of language, our Big City Small World series has lots of conversations between friends and would be a good way of seeing lots of different examples.
