'Weak form' is a term related to pronunciation. It describes vowel sounds which change when they are unstressed.

If you look at the dictionary entry for 'and' (here) you will see the pronunciation given for 'strong' and 'weak' forms. The spelling does not change; this is purely a question of pronunciation in connected speech, and whether or not the rhythm of the sentence requires that the word be stressed or unstressed. Grammar words such as auxiliary words like 'have', common conjunctions like 'and', articles like 'a' tend to be unstressed unless there is a particular reason why they are important in the sentence.
