You might be confusing the meaning with "The gym opens at 4 o'clock in the morning". The verb "open(s)" means "starts to be open", or "starts doing business".
The question says: The gym is open at 4 o'clock in the morning. This is different. "Is open" simply means its status is "open" and not closed, at that moment.
So, taking a different example, if a restaurant opens (i.e., starts doing business) at 12:00 pm, we can say that it is open at 12:01 pm, 12:02 pm, 12:03 pm (etc.) and every moment until it closes.
That's why question 7 is "true". I hope that helps to clarify the answer!

I think it too
Workout is vital for me actually I do it everyday. It makes me stronger physically and mentally . First I used to do my training with an expert to learn me how to do it and when, why to do it i addition to the benefits . After that I became able to do all the exercises by my own. Lots of equipment is important that means no long wait. And don’t waste my time with out practice . Sport complex community around me is a bid motivation to involve all sports events continuously.
Workouts are very important for me, it's a big part of my life and my routine for years. Regular workouts are making me stronger physically and mentally. I like strength trainings and cardio. The good gym aquipment is also very important.

The gym is open at 4 a.m.? Why the answer is True. According to context, it shows the gym is open 24/7, it never close.

'True' is the correct answer here.
When 'open' is a verb it means to stop being closed. For example, if we say 'The shop opens at 7.00' then it means that you can't buy anything before 7.00.
However, when 'open' is an adjective it means not closed. Thus, if we say 'The shop is open 24/7' then it means it is always open. It is open at 1.00, 2.00, 3.00, 4.00 etc.
